IF YOU DO NOT WISH TO RENEW YOUR LEASE THIS FORM MUST BE FILLED OUT 60 DAYS PRIOR TO YOUR LEASE EXPIRING. MONTH TO MONTH TENANTS ARE ONLY REQUIRED TO PROVIDE A 30 DAY NOTICE TO VACATE.
Please be advised that the undersigned hereby provides a written notice to vacate the apartment by the date specified below.
If I plan to vacate the apartment before the date specified below, I will provide my Landlord a new notice to vacate in writing.
I understand that at the time of vacating the final water/sewer bill will be deducted from my security deposit.
I understand that it is my responsibility to remove my name from my account with the Electric Utility Company effective only on the last day of my lease. I AM NOT TO REMOVE MY NAME FROM THE ACCOUNT PRIOR TO THE EXPIRATION OF THE LAST DAY OF MY LEASE.
I understand that my security deposit will be returned, less any necessary deductions (carpet cleaning, final water bill, damages, etc.) within 21 days after the surrender of my premises. If damages exceed the security deposit amount, I will be given written notice of those damages and deductions with 21 days of the surrender of the premises.
The Landlord is hereby authorized to show the premises to prospective residents with proper notification given to the undersigned.
